In case of emergency, I give full consent to those qualified at FAST EMS Training Solutions to perform immediate medical attention and contact my emergency contact.*

I hereby release FAST EMS Training Solutions, and all their employees, from any injury that I may endure on their site throughout the duration of the course. I agree not to pursue any legal action against FAST EMS Training Solutions.*

I will provide FAST EMS Training Solutions with all required documentation.*

I will submit to a ten-panel drug screening and background check.*

I understand that due to purchasing of curriculum, supplies and processing fees, course payments are not fully refundable. The required deposit of $500 will go towards the tuition and is non-refundable. If I withdraw within 14 days prior to the start of my course, any payments I have made to FAST EMS Training Solutions are non-refundable.*

I understand that I may be dismissed from the course for not meeting course requirements or failing to abide by FAST EMS Training Solutions policies and procedures.*
